Public Health nursing

ESDHD has public health nurses on staff to oversee health promotion and disease prevention programs and to assist individuals and families with questions concerning their health. We offer health instruction, information, physician and/or social service referrals, as well as home visits.

ESDHD takes responsibility for assessing the health of the community and ensuring that appropriate control measures are implemented to prevent disease occurrence and/or spread. The health department sponsors a variety of programs within the district to prevent communicable disease, promote wellness and provide health information.

Our Public Health Nurses provide: 

  • Seasonal influenza vaccines for children, adults and high dose for seniors 65+
  • Covid-19 vaccines 
  • Free Tdap vaccines for caregivers of newborns through the CT DPH Cocoon Program
  • Travel Health Immunizations
  • Community Wellness Clinics
  • Health education 
  • Harm Reduction services for overdose prevention, mental health and suicide prevention
  • QPR (Question, Persuade, Refer) suicide prevention training
  • Communicable disease surveillance and follow-up
  • Lead level surveillance, testing follow up, and epidemiological investigations
  • TB surveillance and case management including DOT and EDOT
  • Blood pressure screenings and education 
  • Hemoglobin A1C screenings
  • Linkages to care
  • “Matter of Balance” falls prevention program
  • Flu and Covid-19 vaccine for Homebound residents within our district
  • Overdose education and Narcan training classes in collaboration with our Harm Reduction Navigator
